Your Practices Working Together

Primary Care Network

VENN PCN

Due to changes in the GP Contract, groups of practices are now working together - typically covering between 30-50 thousand patients in an area.
Primary Care Networks build on a core of current primary care services and enable greater provison of proactive, personalised, co-ordinated and more integrated health and social care services.

Primary Care Networks provide the basis for future collaboration with other providers (eg Community Trusts) where appropriate, and will work together to deliver specific packages of care.

OUR PRIMARY CARE NETWORK OFFERS A RANGE OF SERVICES

We are a member of the Venn Primary Care Network (PCN). Together with our  local partner practices we are opening access to a range of services for the benefit of all our patients. Some of these services are available at this surgery, whilst other nw services are conveniently available at local clinics. Some of these services are avilable for patients to self-refer.

Full details of all PCN clinics are available on the PCN website. Additionally, patients will find a wealth of other useful health information and contacts.

Patients are able to self-refer into a range of PCN services. Visit the PCN website Self Referral page for full details of self-refer services:

The Ankle Brachial Pressure Index (ABPI) test is used to diagnose Peripheral Artery Disease. Full details of the symptoms and the test are available on the Venn PCN website. The test can be booked through your practice by calling to discuss your symptoms and the appropriateness of the test.

The Health & Lifestyle Practitioners are non-clinical practitioners located within your GP practice who can support patients with making positive lifestyle changes including improving diet and increasing physical activity levels. The Health & Lifestyle Practitioners focus on the prevention of conditions such as Cardiovascular Disease and Type 2 Diabetes.

Social Prescribing chat

Social Prescribing and community-based support is a key component of personalised care and is part of the NHS Long Term Plan.

Social Prescribing enables GP practices to refer patients to a range of services that offer support for social, emotional or practical needs. Through a link worker, social prescribing connects patients to community groups and services.

Social Prescribing is an all-age, whole population approach that works particularly well for people who:

  • Have one or more long-term conditions
  • Need support with low-level mental health issues
  • Are lonely or isolated
  • Have complex social needs which affect their wellbeing

Extended Hours poster

Extended Access is an extension of your GP practice and offers routine and same day appointments with a range of clinicians including GPs, Nurses and other Healthcare Professionals.

Our practices within the PCN will be able to provide enhanced access appointments between the hours of 6.30pm to 8pm Mondays to Fridays and between 9am and 5pm Saturdays and Sundays.

Please be aware you may not see your regular clinician; all clinicians will have full access to your records and all information following your appointment will be available for your regular surgery to access.

You can contact the surgery to access this service.  Full information is available on the PCN website.

Fractional exhaled nitric oxide (FeNO) is a non-invasive biomarker used in the diagnosis and management of airway inflammation, particularly in asthma. It measures the concentration of nitric oxide (NO) gas in the exhaled breath, which is produced by various cells in the respiratory tract, including epithelial cells, eosinophils, and macrophages.

The test is simple and easy to do and, alongside other diagnostic tests, can tell you and your asthma team what type of asthma you have

A FeNO test is suitable for adults and most children over five.

Full details of FeNO testing is available on the PCN wesbite and appointments can be arranged through the GP practice.

The practice can arrange an appointment for a joint injection. A GP will assess if appropriate for referral, a referral to the Musculoskeletal Clinic may be offered initially. Full information on joint injections is available on the PCN website.

Long-Acting Reversible Contraception (LARC) is a category of contraceptive methods that offer effective, long-term protection against pregnancy with minimal user intervention. In the UK's National Health Service (NHS), LARC methods are promoted as highly reliable options for preventing unintended pregnancies and are readily available to individuals seeking contraception services. More information is available on the Venn PCN website.

Menopause marks the end of a woman's reproductive years and is characterised by the cessation of menstrual periods. It typically occurs between the ages of 45 and 55, although the exact timing varies for each individual. Hormonal changes, specifically a decline in estrogen and progesterone production, lead to various physical and psychological symptoms. Understanding these changes and their implications is essential for women approaching or experiencing menopause.

To arrange an appointment please contact the Practice requesting a referral to the Menopause Clinic. An appointment will be arranged for a blood test. Once your results are available the GP will review these. You will then receive a text message/phone call to arrange your initial appointment.

An extensive mental health portal on the PCN website details the wide-ranging mental health services available for patients.

To arrange an appointment please contact the Practice requesting a referral to the Musculoskeletal (MSK) Clinic. An appointment will be arranged for you to see the GP. The GP will assess you and an appointment will be arranged if appropriate for you to be seen by this service. Please visit the PCN website for full details of the service and clinic times

Venn PCN understands the importance of sleep and how poor sleep can have such a negative impact on health. For that reason, we decided to fund the Sleepstation programme for our patients.  Sleepstation is a personalised online system dedicated to providing resources, support, and guidance to help you achieve better sleep and improve your overall quality of life. Full information is available on the PCN website.

Spirometry is a simple, non-invasive procedure and common diagnostic test used to assess lung function and detect various respiratory conditions, including asthma, chronic obstructive pulmonary disease (COPD), and pulmonary fibrosis. It does so by measuring the volume and flow of air that a person can inhale and exhale. During a spirometry test, a patient breathes into a device called a spirometer, which measures the volume and flow of air expelled from the lungs.

If you feel you have breathing problems please contact the practice to make an appointment with a clinician to plan the best way forward for you.
