NEW PATIENTS
When registering please bring your NHS Medical Certificate to the practice and those of any other persons registering with you.
If you do not have a medical card, you will be asked to complete a registration form for each person registering with the practice.
In addition, all patients will be asked to complete a new patient questionnaire, allowing us to provide medical care in the interim period while your medical records are transferred from your old practice to this one.
If you move out of the practice area, you will need to register with a new practice as soon as you move. We can provide information about how to find a new practice.
In order to register at our surgery you will need to phone to make an appointment for a new patient check with the nurse. You will need to print off and complete the correct forms below and bring them with you to the appointment.
Please note you cannot be registered until you have had your new patient check with the nurse. Therefore please do not post these forms.
Please bring some form of identification - preferably a utility bill, driving licence or passport.

Practices are contractually required to nominate a named accountable GP to take lead responsibility for coordination of services. Patients can express a preference and the practice will make reasonable efforts to accommodate the request.
This improves care coordination. Patients may see any GP. There is an overview of care - and overall care quality is improved.
If you require a specific method of communication, please inform the practice as soon as possible to ensure this information is recorded.

Many migrants will come from countries with very different health care systems to the UK.
It will almost certainly be helpful to explain to new patients how the NHS operates and explore with them how this compares to the healthcare system that they've been used to.
No changes to Dates of Birth can be accepted online - proof must always be provided for these (e.g. a passport or birth certificate). Please attend the Practice to complete a form for this.
Changes of name - other than for reasons of marriage - must also be submitted using a paper form at the Surgery, with proof provided.
·Please remember to inform any other Hospitals, Clinics and other healthcare providers of your change of details if you are currently under treatment with them.
The online Change of Address Form below may be used to change the address or contact details for yourself and other members of your family. Please note that it may take several working days for your details to be updated on our Clinical System
